To obtain the clearance to play all athletes must submit each form that is listed below and return it to the Athletic Training Room prior to participating in any athletic activity.

Athletic Training physical packet, clearance forms and instructions can be found by logging into your Front Rush account. Please reach out your coach to be added to the Front Rush roster.  You will then be emailed a link to your Front Rush account where you can read and complete your online forms for Athletic Training and Compliance clearance.
